TapRooT® Advanced Root Cause Analysis Team Leader
Course Description

The TapRooT® Advanced Root Cause Analysis Team Leader course equips participants with 

the full capability to lead investigations into both precursor and major incidents. The 

training develops strong skills in incident mapping, evidence collection, interviewing, 

causal factor identification, human performance analysis, barrier (safeguard) evaluation, 

and corrective action development. Participants learn and practice all essential TapRooT® 

tools, including SnapCharT®, Safeguards Analysis, the Root Cause Tree®, and the Corrective 

Action Helper®. Optional advanced tools such as Change Analysis, CHAP (Critical Human 

Action Profile), and Equifactor® for equipment troubleshooting are also introduced.

The course blends theory with intensive hands-on exercises, enabling participants to leave 

fully prepared to conduct effective, consistent, and defensible investigations.

By the end of this 5-day program, participants will be able to:

• Lead structured investigations for low-, medium-, and high-risk incidents.

• Apply SnapCharT® to organize evidence and build accurate event timelines.

• Conduct structured interviews and gather physical, documentary, and digital 

evidence.

• Identify Causal Factors and use the Root Cause Tree® to determine real, systemic 

root causes.

• Develop SMARTER corrective actions using the Corrective Action Helper®.

• Apply optional tools—Equifactor®, CHAP, and Change Analysis—when relevant.

• Evaluate safeguard performance and conduct proactive audits.

• Present findings confidently and assess investigation quality.

Day 1 – Fundamentals & Core Tools

• Practical SnapCharT® exercise

• Identifying Causal Factors

• Introduction to the Root Cause Tree®• Root Cause Tree® practice

• Corrective Action concepts & quick exercise

Day 2 – Information Gathering & Interviewing

• Software tips and workflows

• Generic Causes and systemic thinking

• Preparing for investigations

• Advanced evidence collection strategy

• Interviewing techniques

• Two full interviewing role-play exercises

Day 3 – Human Factors & Optional Tools

• Management systems and performance influences

• Human engineering and behavioral contributors

• Introduction to Equifactor® (equipment troubleshooting)

• CHAP (Critical Human Action Profile)

• Change Analysis and practical exercise

• Improving procedures and training analysis

• Case integration practice

Day 4 – Proactive Improvement & Communication

• Human Factors review activities

• Proactive improvement using Safeguards Analysis

• Communications and investigation reporting

• Case exercise (e.g., equipment and human interaction scenario)

• Grading investigation quality

• Presenting to management

• Start of final team exerciseDay 5 – Final Investigation Workshop

BTS Attendance Certificate will be issued after passing 80% of the course duration
